A thoughtful introduction.
A clearer start.
If you are helping someone connect with us, begin with their understanding, permission and preferred way to take part. Initial discussions explore provider collaboration, service fit and referral pathways before participant information is exchanged.

Start with a brief, consented introduction
- Ask about service fit, current availability and the proposed arrangement.
- Confirm consent or relevant authority before information is shared.
- Keep participant records out of the initial website enquiry.

Begin with the type of coordination, the proposed delivery model and the support you need with continuity or caseload. Keep the first discussion at service level.
Before any subcontractor engagement, agree the contracting provider, relevant qualifications and checks, supervision, records, reporting, insurance, fees and escalation responsibilities. Availability and any specialist scope need explicit confirmation.

Participants can contact us themselves. Families, carers and professionals can also make an introduction with the person’s permission or appropriate authority. Clarify this before discussing their circumstances.
Start by providing your own contact details and asking to discuss a potential referral. You can ask about service scope, current capacity, location and provider arrangements without attaching a plan or health report.
An enquiry is not acceptance of a referral or a promise of a start date. We will need to confirm whether the proposed work can be offered and under what arrangements.
Follow the referral stepsIntroduction, fit, information sharing and the next conversation.

The participant’s voice stays visible throughout the introduction.
Agree the introduction
Check that the person understands what Compas Care Coordination is, why you are suggesting contact and how they would like to be involved.
Ask about practical fit
Clarify the kind of coordination, location, current capacity, proposed provider arrangement and any fees.
Agree information sharing
Confirm consent or authority, the information that is needed and a suitable way to exchange it. Share only what is relevant.
Set the next conversation
Agree who will attend, communication or access needs and the next action. Ongoing work requires clear service arrangements.

It can be helpful to clarify which conversations are already taking place, which actions are agreed and which gaps need attention. Separate factual observations from recommendations and pending decisions.
Respect the person’s preferences about involvement and contact. Where someone acts under an authority, agree how this will be established before personal information is exchanged.
If a handover is involved, plan continuity and notice with the existing provider. Avoid assuming that an introduction automatically transfers responsibility for current supports.
